Appointment form
The appointment form runs in your browser. When submitted, it creates a WhatsApp message containing the details you entered. The website itself does not store the form submission on a server.
Contact information
If you contact Ephphatha by phone, WhatsApp or email, those communications are handled through the relevant service and the center's normal communication process.
Cookies and analytics
This starter version does not intentionally set advertising cookies or use paid analytics. If analytics, embedded social feeds, online payments or a patient portal are added later, this notice should be updated accordingly.
Children's information
Please avoid sending highly sensitive health records through the general appointment form. Clinical records should be shared only through a method approved by the center.
